Office Policies



All applicable payments (co-pays, etc.) are expected when services are rendered. We participate with most insurance companies; however, the patient is ultimately responsible for any amounts not paid for by the insurance company. If we do not participate with your insurance company, full payment will be expected at time of service. As a courtesy to you, we will submit the insurance forms to your insurance company. We accept cash, personal checks and most major credit cards.


It is the responsibility of the patient to secure any referrals needed in order to comply with your insurance guidelines. Any necessary referrals need to be in our office at the time of service. You will be responsible for payment of the office visit should you fail to obtain a referral.


Most prescriptions can be renewed by phone. Please request refills only during office hours and have the prescription name and pharmacy number ready.

In order to maintain the highest quality of care, refills cannot be given over the phone to patients who have not been seen in our office within the last year.



If an eye problem is such that you must be seen on an urgent basis, we will make every attempt to see you. If you should need a physician at a time when the office is closed, please call our office and our answering service will contact the on-call physician. If your problem cannot wait, we suggest that you go to the closest emergency room.
